The Department of Veterans’ Affairs (DVA) Community Nursing Program provides in-home nursing and personal care services to eligible veterans and their families. Ludda Care’s highly qualified team delivers expert clinical care and personal support tailored to your assessed needs—helping you remain independent, safe, and comfortable at home, while reducing unnecessary hospital or residential care admissions.

Eligibility for DVA Community Nursing Services

To access DVA-funded community nursing with Ludda Care:

  • Step 01: Check Your DVA Card Type.

    • Veteran Gold Card: Generally eligible for community nursing to meet your assessed clinical and personal needs.
    • Veteran White Card: Nursing support may be approved only for accepted service-related injuries or conditions. Ludda Care will confirm eligibility with DVA before starting your care.
  • Step 02: Get a Professional Referral.

    • Ask your general practitioner (GP), a community nurse practitioner, or your hospital treating doctor/discharge planner for a written referral.
    • Referrals from GPs/nurse practitioners last for 12 months; hospital referrals are usually valid for a shorter time post-discharge.
  • Step 03: Home Assessment.

    • Ludda Care will review your referral and arrange a home visit by a registered nurse.
    • Your nurse will assess your needs, confirm eligibility (especially for White Card holders), and create a care plan tailored for you.
  • Step 04: Start Receiving Services

    • Once eligibility is confirmed, Ludda Care’s community nursing services commence at your home.
